The health and social programs and services available through the Tåîchô Community Services Agency are part of a system of care in the NWT. A person can contact a member of the primary community care team in their home community and receive access to healthcare services in their own community, in the region and, as necessary, outside the NWT.

In our communities, members of these teams are typically nurses, community health representatives, social workers or counselors. The members of these teams work closely with other care givers in the region such as doctors, physiotherapists and other specialists. This model of service is called the Integrated Service Delivery Model or ISDM.

Diagnostic and Curative Services are provided by our primary Community Care Teams in the health centres in Behchokö, Gamètì, Wekweetì & Whatì. People can be assessed and treated for common illnesses, receive first aid and CPR, as well as limited radiology and lab procedures such as blood and urine analysis. Our community staff work with regional and territorial support staff to coordinate referrals for needed services outside the community.
Promotion & Prevention Services help people improve their Nurse health and wellbeing by providing education and information on healthy choices which help prevent illness and injury or to detect and stop problems early. These services are provided by our nurses, Community Health Representatives, Lay Dispenser and other staff. They immunize children to reduce the spread of diseases such as measles, whooping cough, tetanus and chicken pox and adults against the flu virus. They help people to understand the importance of healthy lifestyles by stopping smoking or drinking especially during pregnancy. They do screening programs especially for TB. They also hold well-adult and well-child clinics, do pre and post-natal assessments and help with school health programs and parenting.
Home & Continuing Care Services support people and family caregivers when they can’t fully take care of themselves. The main types of services are Home and Community Care which includes respite care, chronic care, foot care, palliative care, social support and equipment loans; Supported and Facility Living which helps elders to live independently with assistance and help including day programs as well as 24 hour care in a group home
or long term care for people who can’t live at home. Our staff provide home management and personal care to elders, coordinate respite care for family caregivers, offer nutrition programs assist with the upkeep of the home and offer palliative care.

Rehabilitation Services help people improve and nmaintain their independence if they have an injury, disability or chronic disease.
Physiotherapy, Occupational Therapy, Speech and Language Pathology, and Audiology services are all available to residents of the Tåîchô region at the Stanton Territorial Hospital in Yellowknife. People can access these services through a referral from the Community Care Team in their home community.
