Summary: Parents must provide Certificate of Indian Status, or Birth Certificate or Health Care Card to receive payment for their child(ren).To receive payment you must provide proper identification. If you are Sixteen (16) and Older you must provide two pieces of identification as follows to receive payment: Certificate of Indian Status, Birth Certificate, Drivers License, Government Identification Certificate, Firearms Acquisition Certificate, Student  Identification Card, Employee Identification Card, Passport is accepted on their own without a second form of identification.
